Addiction is a complex disease that manifests in various forms, affecting individuals differently. It can involve substance addictions, such as alcohol or drug addiction, or behavioural addictions, such as gambling or internet addiction.
Whilst all forms of addiction have certain elements in common, including:
- An inability to cut down on the substance or behaviour despite the harm it is causing,
- Development of dependence,
- Neglect of usual responsibilities,
- The specific mechanisms and effects can vary widely depending on the substance or activity involved.1,2
Different types of addiction also respond differently to various forms of treatment. Substance addictions, also known as substance use disorders (SUDs), are characterised by the compulsive use of addictive substances, such as alcohol, drugs and prescription medication, despite harmful consequences. These addictions can be severe, leading to physical and mental health issues and an array of negative effects on a person’s life. The Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) recognises substance use disorders arising from different drug classes.1
Alcohol Addiction
Alcohol addiction, alcoholism, or alcohol use disorder (AUD), is a medical condition characterised by an inability to reduce or stop alcohol use despite harmful health and social consequences. Misuse of alcohol can produce changes in the brain which make individuals more susceptible to developing alcohol addiction. Signs of alcohol addiction include needing more alcohol to produce a desired effect (i.e., tolerance), development of physical dependence and presence of withdrawal symptoms when use is reduced or stopped.1,3

Drug Addiction

Many people are first prescribed these medications by their doctors for anxiety, sleep disorders or pain conditions, before developing addiction. Signs of prescription drug misuse include reduced breathing rate, slurred speech and changes in mood.4,5

Behavioural Addictions
Behavioural addictions involve compulsive behaviours that provide a “high” similar to substance addiction. Unlike substance addictions, behavioural addictions do not involve the ingestion of addictive substances but the compulsive need to adopt certain behaviours, even when they lead to significant negative consequences and health issues. These addictions can disrupt a person’s life, causing emotional, physical and social harm.2
Gambling Addiction
Gambling addiction, also known as gambling disorder, is the only behavioural addiction currently recognised in DSM-5. Gambling disorder is characterised by persistent gambling despite negative consequences. Over 95% of people with a gambling disorder also have a mental health condition, such as anxiety or mood disorders. Signs of gambling addiction include:
- Preoccupation with gambling,
- Taking significant risks to achieve greater thrills,
- Financial troubles,
- Lying to family and friends
- Attempting to recoup losses.
- The consequences of gambling addiction can be severe, including financial ruin, job loss, strained relationships and suicide.6
Sex Addiction
Sex addiction, also known as hypersexuality, involves compulsive sexual behaviours that interfere with daily life and relationships. Signs of sex addiction include:
- An excessive preoccupation with sex,
- Obsessive sexual thoughts,
- Participating in risky sexual behaviours
- Investing significant amounts of time in planning, having or recovering from sex.
Although not recognised in DSM-5, sex addiction can be diagnosed as part of other mental health disorders, such as impulse control disorder (ICD).7
Internet Addiction
Internet addiction is not currently recognised in DSM-5 but is a developing field. Subtypes of internet addiction include video game addiction, online gambling addiction and online sex addiction. These disorders are characterised by excessive use of digital devices and online platforms to the detriment of personal, social and occupational functioning.
Signs of internet addiction include excessive internet use, requiring increasing amounts of internet stimulation (i.e., tolerance), the presence of withdrawal symptoms, such as depression and irritability, when not engaged in internet use and neglect of other life areas.8
Food Addiction
Food addiction, also known as compulsive overeating, is a behavioural addiction involving compulsive eating behaviours that persist despite negative consequences. This addiction can result in significant health issues including obesity, diabetes and heart disease. Signs of food addiction include frequent cravings for highly palatable foods, eating to reduce negative emotions and feeling out of control around food.9,10

Co-Occurring Disorders
A co-occurring disorder, also known as dual diagnosis, refers to the simultaneous presence of a substance use disorder (SUD) and a mental health condition. No specific combination is required for diagnosis, i.e., the presence of any mental health condition(s) and SUD(s) is referred to as a co-occurring disorder. Dual diagnosis is common among people in treatment as these conditions are often intertwined and share common risk factors. Mental health conditions, such as anxiety, post-traumatic stress disorder (PTSD) and bipolar disorder, can occur alongside SUDs like alcohol addiction or opioid use disorder. These conditions can result from and contribute to substance misuse, which can generate a vicious cycle.
The co-existence of mental health conditions and SUDs can add complexity to treatment, however, personalised treatment plans can help to effectively manage both conditions concurrently. Treatment for co-occurring disorders can involve medications, behavioural therapies and support groups.11,12
